Create an application form
Create and fill out an application form manually in the Application forms section to collect customer information outside of a Creatio-controlled flow. After you submit the form, Creatio runs a compliance evaluation and synchronizes verified data with the related contact or legal entity record.
Use an individual form to collect data about a person. Use a legal entity form to collect data about a company and the people behind it.

Create an individual application form
Create the form

Go to the Application forms section.

Click New → Individual.

Fill out the required fields on the mini page:
Field name
Description
Contact
The related individual.
Participant role
The ownership/authority role, for example, "Applicant."

Click Save.
As a result, Creatio will add the application form and populate it with data from the linked contact record.
Add identity documents
Enter the documents that confirm the applicant's identity in the Identity documents expanded list. You must upload the document file and fill out the document properties to proceed.
You can add only one active document per class. If an active document of the same class already exists, Creatio blocks the new record and shows a validation message.
A file attachment is required. Creatio does not save the document until you upload a file.
As a result, Creatio will add the document and link it to the application form.
Fill out the Personal data expansion panel
Enter the applicant's biographical details.
Fill out the Communication options expansion panel
Enter the applicant's contact details. Add at least one valid phone number and at least one valid email address.
Add addresses
The Addresses block stores residence, registration, and mailing addresses. Every address includes the Country, State/Province, City, Street, Building, Office/suite, and ZIP/postal code fields.
Follow the case that matches the applicant's situation.
The applicant lives at their legally registered address

Fill out the fields of the Registration address expansion panel.

Click Make primary.

Go to the Residence address expansion panel → Same as primary address.

Answer the Have you lived at your current address more than two years? question.
Click Yes to confirm that no additional address is needed. If the applicant has lived at this address for fewer than two years, click No and fill out the Previous registration address expansion panel.

Fill out the Mailing address expansion panel.
If the mailing address matches the primary address, select Same as primary address to copy the details. Otherwise, enter the address manually.
The applicant lives at an address other than their registered address

Fill out the fields of the Residence address expansion panel.

Click Make primary.

Fill out the fields of the Registration address expansion panel with the official legal details.

Answer the Have you lived at your current address more than two years? question.
Click Yes to confirm that no additional address is needed. If the applicant has lived at the residence address for less than two years, click No and fill out the Previous residence address expansion panel.

Fill out the Mailing address expansion panel.
If the mailing address matches the primary address, select Same as primary address to copy the details. Otherwise, enter the address manually.
As a result, the application form will contain complete address information that distinguishes among the current, registered, and mailing addresses.
Fill out the Primary place of employment expansion panel
This expansion panel is optional if the applicant's Social status field is set to "Unemployed," "Retired," or "Student."
Fill out the Employment address expansion panel
This expansion panel is optional if the applicant's Social status field is set to "Unemployed," "Retired," or "Student."
Submit the form
Submit becomes available after you complete all blocks.
- Click Submit. This opens a mini page.
- Click Submit on the mini page to confirm the action.
As a result, the form status will change to "Submitted" and Creatio will start the KYC evaluation. If the KYC evaluation returns a positive result, the status changes to "Approved" and Creatio synchronizes verified data with the related contact record. After the synchronization, the status changes to "Completed," the Compliance & Risk monitoring tab shows the Evaluation history expanded list, and the form becomes read-only.
Decline the form
Decline is available at any stage.
- Click Decline. This opens a mini page.
- Select a cancellation reason.
- Enter the cancellation reason details (optional).
- Click Save.
As a result, the form status will change to "Cancelled," the form will show the cancellation details, and the form will become read-only.
Create a legal entity application form
Create the form

Go to the Application forms section.

Click New → Legal entity. This opens a mini page.

Fill out the required fields:
Field name
Description
Legal entity
The company associated with the application form.
Participant role
The ownership/authority role, for example, "Applicant."

Click Save.
As a result, Creatio will add the application form and populate it with data from the linked legal entity record.
Add identification documents
Enter the documents that identify the company in the Identification documents expanded list.

As a result, Creatio will add the document and link it to the application form.
Fill out the Legal entity data expansion panel
Enter the company's registration, classification, and business profile details.
Fill out the Communication options expansion panel
Enter the company's contact details. Add at least one valid phone number and at least one valid email address.
Add addresses
The Addresses block stores the company's registration and mailing addresses. Every address includes the Country, State/province, City, Street, Building, Office/suite, and ZIP/postal code fields.

If the mailing address matches the registration address, select Same as registration to copy the details. Otherwise, enter the address manually.
Add ownership structure
Add the company's beneficial owners, authorized signatories, and other related contacts.
At least one ownership structure record is required. If the expanded list is empty, Creatio blocks the submission and shows the "Ownership structure is required." validation message.
As a result, Creatio will link the contact to the application form as part of the company's ownership structure.
Submit the form
After you fill out all required data, you can submit the form for KYB evaluation.
To complete the KYB evaluation, submit the application forms of all contacts in the Ownership structure expanded list and make sure that every form has a positive KYC result.
- Click Submit in the top right. This opens a mini page.
- Click Submit on the mini page to confirm the action.
As a result, the form status will change to "Submitted" and Creatio will start the KYB evaluation. If the KYB evaluation returns a positive result, the status changes to "Approved" and Creatio synchronizes the verified data with the related legal entity record. After the synchronization, the status changes to "Completed" and the form becomes read-only. The Compliance & Risk monitoring tab shows the Evaluation history and the Related application forms expanded lists generated for the ownership structure contacts.
Decline the form

- Click Decline. This opens a mini page.
- Select a cancellation reason.
- Enter the cancellation reason details (optional).
- Click Save.
As a result, the form status will change to "Cancelled," the form will show the cancellation details, and the form will become read-only.
