Application form overview
Application forms in Creatio provide a single, structured way to collect and verify customer data required to open a financial product, onboard a customer, or run a related compliance check. Instead of editing the customer profile directly, users capture and confirm data on the form, and Creatio synchronizes the verified values back to the contact or legal entity record. This keeps every application working from consistent, auditable data and reduces manual re-entry.
Each form is built from pre-configured data blocks, so all required information is gathered in one place, in the same structure, every time.
Application form types
Creatio provides the following types of application forms, depending on for whom the application is:
- Individual application form – captures personal data for a contact. Used for retail onboarding and individual KYC (Know your customer).
- Legal entity application form – captures company data for a legal entity, including its ownership structure. Used for business onboarding and KYB (Know your business).
When you create a form manually in the Application forms section, select the type from the New button menu: Individual or Legal entity.
Form usage flows

Manual processing. Run a customer check whenever you need it, without waiting for an application. Create the form yourself in the Application forms section, for example, for customer-related checks or regulatory and servicing tasks. This flow has the following key features:
- Submit the form when data is ready. Submit is available while the form is in the "Draft" status. Creatio validates the required fields when you click Submit, so the action remains visible even if required data is still missing and you can see exactly what to fill out. Submission changes the status to "Submitted," locks the form, and completes its processing.
- Get the evaluation without launching it manually. On submission, Creatio automatically launches the required evaluation: KYC for an individual form, KYB for a legal entity form. When the evaluation completes successfully, verified data is synchronized automatically to the linked contact or legal entity record, so you do not need to copy it over.
- Stop processing at any point before submission. Decline changes the status to "Cancelled" and locks the form.
- Describe the person's actual part in the process. Manually created forms are not restricted to the "Applicant" participant role. Select any role that fits, for example, "Borrower."

Creatio-controlled flow. Skip form creation entirely in pre-built application flows. Creatio generates an application form for the "Applicant" participant role, links it to the corresponding application, and processes it as part of the application lifecycle. This flow has the following key features:
- No manual submission needed. Submit is not available in this mode, as the form is submitted automatically during the application workflow.
- Cancel the whole chain in one step. Decline functions in the same way as in manual processing. If no other application forms exist for the same applicant, the linked application is also declined.

Commercial sales. Verify a company's data and ownership structure (KYB) without leaving the opportunity. In the commercial sales process, Creatio generates a legal entity application form for the opportunity so that the check happens directly within the B2B deal flow. The form is linked to the opportunity and the Application field remains empty, so you keep full control: both Submit and Decline are available while the form is in the "Draft" status, as in manual processing.
Data synchronization
Creatio automatically synchronizes data between application forms and the underlying customer records, both contacts and legal entities, to ensure data accuracy, reduce manual entry, and support compliance. All updates to verified customer data are managed through application forms, keeping the contact or legal entity profile consistent and traceable throughout the application process.
